About this role
Eldercare is a leader in Palliative Care in Aged Care. It recognises excellence in palliative care as an integral part of care provision to residents and seeks to continue to drive this through Eldercare’s Palliative Approach, systematically supporting and implementing contemporary clinical practice for residents that are palliating or at the end of life.
Eldercare is now very excited to be expanding their Palliative Care team and service provision offering to sites through the addition of either:
- An Endorsed Nurse Practitioner, with extensive experience working and specialising in Palliative Care.
- Specialist Palliative Care Nurse or
- Nurse Practitioner Candidate, that has completed palliative care studies and either commenced or is seeking to commence training as a Nurse Practitioner.
This specialist role will join Eldercare’s existing Palliative Care Team and work to build competence and capability of multi-disciplinary team members to deliver effective palliative care, which is aligned to our ‘Model of Care’.
This role will use a multidisciplinary approach to continue to support and drive excellence in palliative care at Eldercare’s residential care homes throughout South Australia and will collaborate with Eldercare clinical, wellbeing and allied health teams to ensure our residents receive a comfortable and dignified experience, that respects their wishes and choices.
The role will also:
- Provide comprehensive assessment of palliative residents, recommending management strategies.
- Undertake monthly palliative care needs rounds across Eldercare sites and assist to identify residents who are at the end of life
- Promote advanced care planning for all residents.
- Promote collaboration with GPs and specialist services.
- Facilitate access to Social Worker and other multi-disciplinary support.
- Promote support to clinical and care staff in complex palliative end of life care.
- Reporting to the Specialist Nurse Manager, and working closely with Eldercare’s multidisciplinary team, the role will continue to drive Eldercare’s Palliative Approach to care.
To succeed in this role, you will need:
- Demonstrated ability to lead and promote best practice palliative care.
- Ability to develop and foster high level skills in managing residents with palliative care
- Ability to build confidence and capability of clinicians and multi-disciplinary team members through effective coaching and mentoring
- Ability to partner effectively with internal and external partners to achieve identified organisational outcomes based on evidence-based practice.
- Excellent communication and interpersonal skills with the ability to effectively engage and partner with consumers, families, and staff of diverse ages, cultures, and backgrounds.
- Ability to work collaboratively, lead and contribute to continuous improvement.
- High level stakeholder engagement and consultation skills.
- Be willing to travel as required
Qualifications and Experience:
- Relevant and broad experience as a Registered Nurse
- Registration as an Endorsed Nurse Practitioner with AHPRA with extensive experience working in palliative care AND Endorsed by the Nursing and Midwifery Board of Australia (NMBA) as a Nurse Practitioner OR
- Specialist Palliative Care Nurse with AHPRA registration OR
- Nurse Practitioner Candidate, that has commenced palliative care studies or is seeking to commence training as a Nurse Practitioner.
- Current Registration with the Nurses Board of South Australia
